Waiver Liability Form

I am aware that participation in the Dunk Dynasty Basketball League has some inherent risks and that injuries can occur. On occasions, these injuries can be serious. In consideration of these risks, I am acknowledging that i assume all risk and agree not to sue Dunk Dynasty Basketball League, it's owners, participants, volunteers, coaches, directors, agents, or the facility for any injuries or concerns caused by OR resulting from participating in the Dunk Dynasty Basketball League. By signing this waiver, I also authorize the use of all pictures of the participant to be posted on any media within the context of Dunk Dynasty Basketball League, which may include; Instagram, Facebook, Website content or any advertising media published by the league.

By submitting your information, you're giving us permission to email you and contact you if needed. please note all team sheets are final except for the case of an injury or player drop out. the person signing up their team will be responsible for making the team payment towards dunk dynasty via e transfer to dunkdynastybasketball@outlook.com.
